Can I still see my dentist for care if he/she doesn't participate with United Concordia's network?

You can receive care from any licensed dentist, anywhere in the United States. If you choose to see a nonparticipating dentist, you will be responsible for the coinsurance amount that is part of your program design as well as any charges over and above United Concordia's allowance for covered services. You may also have to fill out claim forms and accept payment from United Concordia.

Do I have to fill out claim forms after each visit?

If you receive care from a United Concordia participating dentist, you need not worry about claim forms - your dentist will take care of all the paperwork. If you receive care from a nonparticipating dentist, then you may have to fill out a claim form.

How will my dentist know I'm a member of Concordia Flex?

Once you have enrolled in the Concordia Flex dental plan, you can print out an identification card from our website using our My Dental Benefits feature. In addition, you or your dentist can contact our customer service center to confirm coverage.

Does United Concordia require predetermination of benefits?

While predeterminations are not required, you should consider asking your dentist to provide a predetermination, or pre-treatment estimate, before undergoing any type of extensive treatment. That way, you'll know if the service is covered an exactly what your financial responsibility will be.

How do I sign up for Concordia Flex?
