DENTAL HMO PLAN

United Concordia is pleased to serve the dental insurance needs of PEBTF members. PEBTF and United Concordia are partnering because we care about the oral health of you and your family. Oral health problems often tax the immune system, which can expose you to greater risk of illness and infection. To help maintain a healthier smile and a healthier you, we encourage you to follow the advice of your dentist and the American Dental Association. Your dental plan emphasizes preventive care to help avoid problems commonly caused by delayed treatment.

About My Plan - Your dental Plan is the Concordia Plus Dental HMO (DHMO) Plan. As a DHMO member, you must select a primary dental office to provide and coordinate all your dental care, including specialty referrals. There are no benefits available out-of-network. This plan reduces your out-of-pocket costs as it features no deductibles and no annual maximum.

How Does My Dental Plan Work? - 

• Members, including dependents, MUST select a primary dental office from the Concordia Plus network to provide and coordinate all your dental care. There are no benefits available out-of-network.
  
  
• Preventive and diagnostic dental care is covered in full, while restorative and other major services are offered at a reduced cost.
  
  
• There are no claim forms and you are only responsible for copayment amounts which are part of the program design; however, claim submission is necessary for out-of-area emergency care.
  
  
• You MUST obtain a referral form from your Primary Dental Office to see a specialist.
