FedEx Ground/FedEx SmartPost
Concordia Preferred is United Concordia's preferred provider organization (PPO). A dental PPO combines the cost savings associated with managed dental care programs with the greater access available in traditional Fee-for-Service programs. With the FedEx Ground/FedEx SmartPost PPO, any licensed dentist may provide services. However, there may be a reduction in benefits for services provided outside of the network.
In-network (or participating) dentists agree to:

- Accept United Concordia's fee as payment-in-full for covered services.
- Abide by our quality review decisions.
- File claims on your behalf.

This means that you are responsible only for those deductibles and coinsurance amounts that are part of the FedEx Ground/FedEx SmartPost PPO program. This amount may vary depending upon whether a participating or non-participating provider performs the services.
