PPO MAC Schedule Notice

United Concordia has finalized the annual review of our PPO Maximum Allowable Charge (MAC) schedules for both the National Fee-for-Service and Concordia Parnet Advantage networks. The review included an independent analysis by region, which resulted in updates to some schedules. Please note that not all schedules received an increase. In the affected regions, the updates are applicable for services rendered on or after February 1, 2009 for all groups administered using these networks. Please refer to your specific schedules for additional information.

To download a copy of the updated schedules, go to My Patients' Benefits on the left side of your screen. Select the “Log In Now!” button. Once you are logged into My Patients' Benefits and have entered specific patient information, select the Allowance/Copay tab. Next, enter a procedure code and charge amount in the appropriate fields and click Retrieve.

If you are a United Concordia participating provider, the schedule is available online. The message “Click here to view the entire Maximum Allowable Charge Schedules” will appear. You must have Adobe Acrobat Reader version 6.0 or higher to download the schedule.
