Obtaining a Provider Number
Participating Providers
When you signed up to participate with United Concordia, you were automatically assigned a Provider Number. If you have not yet received your welcome packet with your Provider Number, please contact our Customer Service department at 1-800-332-0366 and someone will assist you. Otherwise, you can locate your provider number in the welcome packet materials.
Nonparticipating Providers
If you are not currently participating with United Concordia and would like to join, click here for an application. If you are not interested in joining at this time, there are still two other ways you can be assigned a Provider Number.
The first way to receive a Provider Number is to send in a claim. When United Concordia receives a claim from a nonparticipating provider, a Provider Number is automatically assigned in order to process the claim though out the system. If United Concordia has already processed a claim for you, then you already have an assigned number. Please call our Customer Service department at 1-800-332-0366 to obtain this number.
If you have never sent us a claim, then you can obtain a Provider Number by completing the following steps:
- Open and Print the Dental Provider Application.
- Complete the required information.
- Send us the form via fax to (717) 260-6834.
