Please check with your plan administrator to determine if the option for each family member to select a different dentist is included in your plan.

How does Concordia Plus assure quality?
Our quality assurance program is a hands-on process of office reviews and regular contact with network providers.

Do I have to fill out claim forms after each visit?
No. There are no claim forms to worry about. The participating dental office will submit the appropriate paperwork.

No. There are no waiting periods because there are no pre-existing condition clauses, except for orthodontia treatment in progress.

Are there any benefit maximums?
No. Under Concordia Plus, there are no dollar limits for in-network services.
